Enrolling At The Center

To be enrolled at Maple Hill a child must be between 3 years and 6 years of age and be toilet trained. Prior to admission each parent and child should visit the center and meet the staff. The next step is a meeting to fill out the enrollment application including Emergency Treatment Form and Authorization For Release Form and review the centers policies.

Before beginning at the center we must have your child’s up to date immunization record, proof of a physical examination, and record of lead screening. Please remember to give us any new immunization or physical records as they occur throughout the year. All enrollment forms will be updated annually.
